Module description

The impact of NHS reforms of recent years (Community Care Act 1990, GMC Contract 2002, the New NHS Primary Care Act 1997, NGMS Contract 2003, Choosing Health 2004) has led to an increase in the provision of primary health care services. Demand on these services includes an increase in patients who request a same day appointment for minor ailments. In hospitals and institutions, nurses and allied health professionals (AHP) have to undertake additional complex interventions and roles in order to delivery quality care to a range of clients in a variety of settings. Nurses and AHPs employed in a variety of healthcare settings can, with appropriate development of their knowledge and skills, offer the potential to provide same day consultation services for patients. Many primary health care nurses already diagnose and treat patients who have minor ailments, however, it is fair to say that this development has not always been underpinned and supported with the appropriate education and skills acquisition. Professional, ethical and regulatory issues are inherent in this development of the health professional's role. This module, therefore, offers nurses and AHPs the opportunity to enhance their existing knowledge and skills through a programme that integrates theory and practice in order to acquire the competencies required to provide a same day consultation service for patients. Ongoing support and supervision is integral to the role development as is audit to ensure safe effective practice to plan future development. Placement in clinical practice must be supported by a named medical supervisor (staff grade or above) or an appropriately experienced/qualified advanced practitioner (agreed by the manager and module tutor).

The clinical assessment process Symptom-based, patient safety-focused clinical assessment Face-to-face and telephone consulting Assessment of patients with ENT, respiratory, cardiovascular, musculoskeletal, neurological, skin, eye, gastro-intestinal and genito-urinary symptoms. Use of the appropriate NICE guidelines Recognition of 'red flag' findings Investigation and referral pathways Risk assessment and safety-netting Documenting a clinical assessment Evidence-based and reflective practice
